ABSTRACT

La sarcopenia está caracterizada por la baja masa y fuerza muscular en los adultos mayores. La implementación de técnicas de modificación de la conducta como la técnica de economía de fichas (TEF) puede fungir como estrategia para mejorar la adherencia al tratamiento. El objetivo fue evaluar el efecto de la TEF sobre las conductas de tratamiento: realizar la rutina de ejercicio de resistencia, consumir el suplemento de proteína y consumir el suplemento de vitamina D. Se realizó un estudio de caso de múltiples sujetos en ocho adultos mayores con sarcopenia. El estudio estuvo conformado por tres fases: pre-intervención, intervención conductual y post-intervención conductual. Se evaluó la frecuencia de las conductas de tratamiento en las tres fases, además de los cambios en masa y fuerza muscular. El análisis de los datos se realizó por medio de estadística descriptiva. Los resultados mostraron que la frecuencia de las conductas de tratamiento aumentó en el 100% de los participantes en la etapa de intervención conductual al comparar con la fase pre-intervención, valores que no regresaron a cero en la fase post-intervención conductual. El 83.3 % de los participantes dejaron de tener el diagnóstico de sarcopenia con base en los cambios de masa y fuerza muscular.


Sarcopenia is characterized by low muscle mass and strength in older adults. It is caused by decreased physical activity and quality of dietary intake. Therefore, the implementation of behavior modification techniques such as the token economy technique can serve as a strategy to improve treatment. The aim of the current study was to evaluate the effect of the token economy technique on treatment behaviors. A multiple case studies with an ABC-type design was conducted in eight older adults with sarcopenia. The intervention consisted of modification of treatment-related behaviors; in addition, changes in muscle mass and strength were assessed. The results showed that the frequency of target behaviors increased in 100% of the participants in the intervention stage compared to baseline. The frequency of the target behaviors did not return to the obtained baseline compared to the maintenance stage. 83.3% of the participants changed their sarcopenia diagnosis.

ABSTRACT

Introduction: the public health crisis caused by COVID-19, a disease caused by SARS-CoV-2, has imposed physical threats and psychological suffering on both infected patients and individuals who experience social isolation and various governmental restrictions, leading to the appearance of symptoms like anxiety, depression, as well as insomnia, stress, and changes in biological rhythm. In face of this stressful scenario, interventions based on mindfulness (MBIs) have proved to be potentially adequate tools in reducing psychological suffering and generating well-being in the general population.Objective: to describe the effects of mindfulness-based interventions during the COVID-19 pandemic. Methods: A systematic literature review was conducted on the effects of mindfulness intervention in times of COVID-19. The articles were searched in four databases (Pubmed, Embase, Scopus, and Science direct) and the PRISMA protocol was used to conduct this review. In total, fourteen articles were included in the study.Results: the use of mindfulness techniques in the population with impaired mental health because of the COVID-19 pandemic proved to be beneficial, with improvement in emotional stress scores and reduction in anxiety symptoms, through formal mindfulness meditation practices such as mindful breathing, body scanning, and application of the mindfulness-based stress reduction strategy (MBSR). Strategies were also applied through smartphone applications that had the objective of promoting the increase of mindfulness and the development of the acceptance without judgment of the traumatic experiences already lived, in addition to an integrated intervention on the internet with the use of mindfulness, the training of respiratory relaxation, refuge skills and Butterfly Hug Method.Conclusion: with the pandemic going through, still with high mortality, continuity of social isolation, and, consequently, intensification of psychic suffering in the population and health professionals, interventions based on mindfulness (MBIs) are being effective to reduce this suffering, prevent the appearance of of chronic mental disorders and promote positive impacts on physical and mental well-being.


Introdução: a crise de saúde pública ocasionada pela COVID-19, doença causada pelo SARS-CoV-2, impôs ameaças físicas e sofrimento psíquico tanto aos pacientes infectados quanto aos indivíduos que vivenciam o isolamento social e as diversas restrições governamentais, propiciando o aparecimento de sintomas de ansiedade, depressão, além de insônia, estresse e alterações do ritmo biológico. Diante desse cenário estressor, as intervenções baseadas em atenção plena (MBIs), se mostraram ferramentas potencialmente adequadas na redução do sofrimento psicológico e na geração de bem-estar da população em geral.Objetivo: descrever os efeitos das intervenções baseadas em mindfulness em tempos da COVID-19.Método: foi realizada uma revisão sistemática da literatura sobre os efeitos da intervenção mindfulness em tempos da COVID-19. Os artigos foram pesquisados em quatro bases de dados (Pubmed, Embase, Scopus e Science direct) e o protocolo PRISMA foi utilizado para realização desta revisão. No total, quatorze artigos foram incluídos no estudo.Resultados: a utilização de técnicas de mindfulness na população com prejuízos na saúde mental em consequência da pandemia da COVID-19 mostraram-se benéficas, ocorrendo melhora no escore de estresse emocional e redução dos sintomas de ansiedade, através de práticas formais de meditação tipo mindfulness, como respiração consciente, escaneamento corporal, e aplicação da estratégia de redução do estresse baseada em atenção plena (MBSR). Estratégias também foram aplicadas através de aplicativos de smartphone que tiveram o objetivo de promover o aumento da atenção plena e o desenvolvimento da aceitação, sem julgamentos, das experiências traumáticas já vividas, além de intervenção integrada na internet com o uso da atenção plena, treinamento de relaxamento respiratório, habilidades de refúgio e método do abraço de borboleta. Conclusão: com o perpassar da pandemia, ainda com elevada mortalidade, continuidade dos isolamentos sociais, e, consequentemente, intensificação do sofrimento psíquico na população e profissionais de saúde, as intervenções baseadas em atenção plena (MBIs) estão sendo eficazes para diminuir esse sofrimento, prevenir aparecimento de transtornos mentais crônicos e promover impactos positivos no bem-estar físico e mental.

Resumen: El trabajo de campo etnográfico, que implica el encuentro con la otredad, es utilizado por la antropología médica para acercarse a la comprensión de problemáticas sociales relacionadas con la salud, la enfermedad, la atención y la muerte. El objetivo del trabajo fue realizar una reflexión ética de diversas situaciones conflictivas experimentadas por tres antropólogas durante sus investigaciones en México, a través del análisis de sus diarios de campo. Con este fin, se realizó un análisis grupal e interdisciplinario desde una perspectiva ética. Los conflictos encontrados se clasificaron en: 1. Dilemas éticos (como la sinceridad vs. ocultamiento de información); 2. Disonancias éticas (como la justicia en el acceso de los informantes a los servicios médicos, el acceso y la identidad del antropólogo en campo, los límites de la intervención, la reciprocidad y las emociones del investigador); y 3. Otros (conflictos éticos tales como la observación del paternalismo médico y las diferencias disciplinares entre el quehacer antropológico y el médico). Este análisis permitió comprender a través de una alerta ético-metodológica los posicionamientos epistemológicos, metodológicos y, principalmente, éticos de las investigadoras que, de manera subrepticia, guían la construcción del quehacer antropológico. Asimismo, la investigación permitió vislumbrar la responsabilidad de las acciones o inacciones del investigador frente a las personas observadas dentro de un contexto de atención en salud.


Abstract: Ethnographic fieldwork, which implies the encounter with otherness, is used by medical anthropology to approach the understanding of social problems related to health, illness, care and death. The objective of the work was to carry out an ethical reflection on various conflictive situations experienced by three anthropologists during their research in Mexico, by means of the analysis of their field diaries. To this end, a group and interdisciplinar y analysis was carried out from an ethical perspective. The conflicts found were classified as: 1. Ethical dilemmas (such as sincerity vs. concealment of information); 2. Ethical dissonances (such as fairness in informants' access to medical services, access and identity of the anthropologist in the field, limits of the intervention, reciprocity, and the emotions of the researcher); and 3. Others (ethical conflicts such as the observation of medical paternalism and the disciplinary differences between anthropological and medical work). This analysis allowed us to understand, through an ethical-methodological alert, the epistemological, methodological and, mainly, the ethical positions of the researchers who, surreptitiously, guide the construction of the anthropological task. Likewise, the investigation allowed us to glimpse the responsibility of the actions or inactions of the researcher vis a vis of the people observed within a context of health care.


Resumo: O trabalho de campo etnográfico, que implica o encontro com a alteridade, é utilizado pela antropologia médica para abordar a compreensão dos problemas sociais relacionados à saúde, doença, cuidado e morte. O objetivo do trabalho foi realizar uma reflexão ética sobre diversas situações conflituosas vivenciadas por três antropólogos durante suas pesquisas no México, por meio da análise de seus diários de campo. Para tanto, realizou-se uma análise grupal e interdisciplinar a partir de uma perspectiva ética. Os conflitos encontrados foram classificados em: 1. Dilemas éticos (como sinceridade x ocultação de informações); 2. Dissonâncias éticas (como equidade no acesso dos informantes aos serviços médicos, acesso e identidade do antropólogo em campo, limites de intervenção, reciprocidade e emoções do pesquisador); e 3. Outros (conflitos éticos como a observação do paternalismo médico e as diferenças disciplinares entre o trabalho antropológico e o médico). Essa análise permitiu compreender, por meio de um alerta ético-metodológico, as posições epistemológicas, metodológicas e, principalmente, éticas dos pesquisadores que, sub-repticiamente, orientam a construção da tarefa antropológica. Da mesma forma, a investigação permitiu vislumbrar a responsabilidade das ações ou omissões do pesquisador diante das pessoas observadas em um contexto de atenção à saúde.

Resumo Objetivo: como profissão orientada à atenção da saúde, focada no cuidado às pessoas, a Enfermagem precisa desenvolver habilidades de comunicação sólidas. Com base em um consenso internacional de especialistas, apresenta-se uma proposta sobre os resultados de aprendizagem na comunicação clínica para planos de estudos em cursos de Enfermagem em países hispanofalantes. Método: um comitê orientador, composto por 5 enfermeiros e especialistas em comunicação em ciências de atenção à saúde, elaborou a primeira lista de habilidades comunicacionais específicas para o curso de Enfermagem. Outro comitê de 7 assessores científicos internacionais revisou e melhorou a referida proposta. Foram selecionados 70 especialistas de 14 países pelo procedimento de amostragem "bola de neve", sendo convidados para participar em um processo de consenso Delphi modificado a distância em duas rodadas de pesquisa. Foi realizada a análise estatística para estabelecer o nível de consenso final correspondente a cada item. Resultados: foi apresentado um questionário com 68 resultados de aprendizagem em comunicações clínicas para serem avaliados pelo painel. Na primeira rodada Delphi, o painel chegou a um consenso estatístico em todos os itens avaliados. Não foi necessária uma segunda rodada para conciliar posições. Conclusão: apresenta-se uma proposta acadêmica aprovada com um elevado nível de consenso internacional a fim de orientar e unificar os resultados de aprendizagem nos planos de estudo sobre comunicação clínica para cursos de Enfermagem em países hispanofalantes.


Abstract Objective: as a health care profession focused on caring for people, Nursing requires sound communication skills. Based on an international expert consensus, a proposal on learning outcomes in clinical communication for undergraduate Nursing education curricula in Spanish speaking countries is presented. Method: a steering committee, consisting of 5 nurses and experts in communication in health care sciences, drew up the first list of communication skills specific to the Nursing degree. Their proposal was reviewed and improved by a committee of 7 international scientific advisers. 70 experts from 14 countries were selected using a snowball sampling procedure and invited to participate in a distance modified Delphi consensus process in two survey rounds. Statistical analysis was carried out to establish the final consensus level for each item. Results: a questionnaire with 68 learning outcomes in clinical communications was submitted for panel assessment. In the first Delphi round, the panel reached a statistical consensus on all the items assessed. There was no need for a second round to reconcile positions. Conclusion: an academic proposal, approved by a high level of international consensus, is presented to guide and unify the learning outcomes on the clinical communication curriculum for undergraduate Nursing studies in Spanish speaking countries.


Resumen Objetivo: en su carácter de profesión de atención de la salud enfocada en cuidar a las personas, la Enfermería requiere habilidades de comunicación sólidas. Sobre la base de un consenso internacional de expertos, se presenta una propuesta sobre resultados de aprendizaje en la comunicación clínica para planes de estudio de las carreras de grado de Enfermería en países de habla hispana. Método: un comité orientador, compuesto por 5 enfermeros y expertos en comunicación en ciencias de atención de la salud, elaboró la primera lista de habilidades de comunicación específicas para la carrera de Enfermería. Un comité de 7 asesores científicos internacionales revisó y mejoró dicha propuesta. Se seleccionó a 70 expertos de 14 países a través del procedimiento de muestreo "bola de nieve", y se los invitó a participar en un proceso de consenso Delphi modificado a distancia en dos rondas de encuesta. Se realizó un análisis estadístico para establecer el nivel de consenso final correspondiente a cada ítem. Resultados: se presentó un cuestionario con 68 resultados de aprendizaje en comunicaciones clínicas para que lo evaluara el panel. En la primera ronda Delphi, el panel llegó a un consenso estadístico en todos los ítems evaluados. No fue necesaria una segunda ronda para conciliar posiciones. Conclusión: se presenta una propuesta académica, aprobada con un elevado nivel de consenso internacional, a fin de orientar y unificar los resultados de aprendizaje en los planes de estudio sobre comunicación clínica para carreras de grado de Enfermería en países de habla hispana.

Introducción: La motivación, el aprendizaje y la ejecución, presentan una relación recíproca según las últimas investigaciones en aprendizaje autorregulado. La motivación influye en el aprendizaje, los estudiantes organizan y elaboran mejor los materiales, se esfuerzan más, piden ayuda con más frecuencia y en definitiva están más comprometidos con la tarea académica. Por otro lado, la ejecución y lo que los estudiantes aprenden y realizan, puede, a su vez, aumentar o disminuir la motivación, a través de variables como la autoeficacia. Objetivo: Identificar qué variables motivacionales y cognitivas predicen el rendimiento académico y como lo hacen. Métodos: Se realizó un estudio descriptivo e inferencial con una muestra de 508 estudiantes de ciencias de la salud pertenecientes a dos universidades privadas españolas, a la que se evalúa a través del Motivated Strategies For Learning Questionnaire, la utilización de estrategias cognitivas, metacognitivas, de gestión de recursos y motivacionales; después, se realizó un modelo de regresión, tomando estas estrategias como variables independientes y el rendimiento medido a través de las notas de los estudiantes como variable dependiente. Resultados: La autoeficacia para el rendimiento fue la variable que mejor predijo el rendimiento académico en los estudiantes universitarios de la muestra. Otras variables que también se relacionaron con el rendimiento fueron la gestión del tiempo y el lugar, la búsqueda de ayuda y la meta de autoensalzamiento. Conclusiones: Los resultados académicos dependen de los métodos de evaluación, del uso de estrategias cognitivas, metacognitivas, y motivacionales adecuadas al contexto instruccional, y de una mejor gestión de los recursos de aprendizaje. Estos hallazgos nos orientan a los docentes a proponer intervenciones educativas dirigidas a aumentar el uso de estrategias motivacionales y de aprendizaje en estudiantes universitarios(AU)


Introduction: Motivation, learning and practice have a reciprocal relationship, according to latest research in self-regulated learning. Motivation influences learning, students organize and elaborate materials better, make greater effort, ask for help more frequently, and ultimately are more committed to the academic task. On the other hand, the execution and what students learn and do in practice, can in turn increase or decrease motivation, through variables such as self-efficacy. Objective: To establish what motivational and cognitive variables predict academic performance and how they do it. Methods: A descriptive and inferential study was carried out with a sample of 508 students of health sciences belonging to two Spanish private universities, which are evaluated, through an adapted Motivated Strategies for Learning Questionnaire, their usage of cognitive and metacognitive strategies, and others for resource management and associated with motivation. Later, we elaborated a regression model, taking these strategies as independent variables and the performance measured through the student's grades as a dependent variable. Results: Self-efficacy for performance was the variable that best predicted academic performance in the university students that made up the sample. Other variables that were also related to performance were the management of time and place, the search for help, and the goal of self-praise. Conclusions: The academic results depend on the evaluation methods, the use of cognitive, metacognitive, and motivational strategies appropriate to the instructional context, and on better management of learning resources. These findings guide teachers to propose educational interventions aimed at increasing the use of motivational and learning strategies in university students(AU)

ABSTRACT

Objective: This parallel, randomized, examiner-blind clinical trial aimed to evaluate the efficacy of two different toothbrushes (manual and sonic) on plaque control in adolescents. Material and Methods: This study enrolled 56 volunteers, randomly allocated to two different groups: group A (n = 28) for the manual toothbrush (Curaprox 5460 Ultra Soft®) and group B (n = 28) for the sonic toothbrush (Edel White®). Mean age was 17.2 ± 1.1 years. A calibrated periodontist performed a periodontal evaluation to assess the presence of plaque with the Turesky Modification of the Quigley Hein Plaque Index (PI) and the gingival inflammation with the Silness & Löe Gingival Index (GI). Adolescents received instructions about the mechanical control of plaque at baseline (T0), with a reexamination after 3 months (T1) and 6 months (T2). Non-parametric Mann Whitney test was used to compare the differences between the two toothbrush groups and the Friedman test was used for the comparisons between times. Results: There has been PI reduction concerning the study times (T0, T1 and T2, p<0.05), but not between the groups A and B (p>0.05). As for GI there has been no significant difference between the groups and between the study times (p>0.05). Conclusion: Both toothbrushes were efficient in the control of supragingival plaque (visible biofilm).

ABSTRACT

Resumen El consumo de sustancias entre jóvenes y adolescentes representa un importante problema de salud que se relaciona con lesiones y accidentes graves, discapacidad, trastornos por consumo de sustancias y otros trastornos psiquiátricos, ideación y conducta suicida, conductas sexuales de riesgo, entre otros. Se trata de un problema de salud pública que se ha incrementado de manera considerable en los últimos años. El objetivo de este artículo es revisar los aspectos generales del consumo de sustancias de abuso en adolescentes, como: epidemiología, factores de riesgo, tamizaje, evaluación, diagnóstico, algunas consideraciones para la referencia y tratamiento en un hospital general. Como resultado, se identificó que existen múltiples factores de riesgo del inicio del consumo de sustancias, así como de la aparición de un trastorno por consumo de sustancias. Entre éstos destacan factores individuales, familiares y medioambientales. Asimismo, se identificaron estrategias para la detección temprana, orientación y referencia a tratamiento integral, conocidos como SBIRT (de sus siglas en inglés de Screnning, Brief Intervention, Reference to Treatment). Por último se concluye que la implementación de programas SBIRT en un hospital general es eficaz para mejorar el vínculo entre los servicios del hospital y los programas especializados para tratamiento de los trastornos por consumo de sustancias.


Abstract Substance abuse among young adults and teenagers represents a considerable health issue, as substance use can be associated to serious injuries, accidents, disabilities, substance use and other psychiatric disorders, suicidal ideation and suicidal behavior, sexual risk practices, among others. In the last years, this health problem has been increasing significantly. The objective of this article is to present a review of general aspects of substance abuse in adolescents, such as: epidemiology, risk factors, screening, evaluation, diagnosis, some considerations for referral and general hospital treatment. As a result, it was found that several risk factors have been identified, such as individual, family and environmental factors. Also, strategies for early detection, counsel and reference to integrated treatment known as SBIRT (Screnning, Brief Intervention, Reference to Treatment) were identified. Finally, it is concluded that the implementation of SBIRT programs in general hospital are effective to improve the link between general hospital services to specialized programs for the treatment of substance use disorders.

Introducción: una de las herramientas que aumenta tanto el interés como el valor de la tarea es el uso del mapa conceptual, que utiliza la representación visual de conceptos relacionados entre sí a través de interconectores. Objetivo: comparar si el uso de estos mapas conceptuales mejora el rendimiento académico y si existen diferencias en el rendimiento en relación al turno académico asignado. Métodos: para ello se seleccionaron de manera aleatoria a 200 alumnos de Grado de Enfermería (Centro Universitario de San Rafael Nebrija-Madrid-España). Resultados: los análisis realizados indican que las medias en cuanto al rendimiento académico son más elevadas en el grupo de alumnos que usaron mapas conceptuales, encontrándose diferencias significativas respecto a los que no lo usaron (t= -2,43; gl: 198; p> ,05). Por otro lado, se rechazó la influencia del turno asignado sobre el rendimiento académico. Conclusiones: el uso de los mapas conceptuales produce un efecto positivo en el rendimiento académico, lo que nos plantea la necesidad de realizar una docencia teórico-práctica más centrada en este método de aprendizaje(AU)


Introduction: One of the tools that increases both interest and value of the task is the use of conceptual maps, which uses the visual representation of concepts related to each other through interconnectors. Objective: To compare whether the use of these conceptual maps improves the academic performance and if there are differences in the performance in relation to the assigned academic shift. Methods: 200 students for the Nursing Degree (University Center of San Rafael Nebrija, Madrid, Spain) were randomly chosen. Results: The analyses carried out indicate that the means in terms of academic performance are higher in the group of students who used conceptual maps, finding significant differences in relationship to those who did not use them (t= -2.43; gl: 198; p> .05). On the other hand, the influence of the assigned shift on academic performance was rejected. Conclusions: The use of conceptual maps produces a positive effect on the academic performance, which raises the need for a theoretical-practical teaching centered on this method of learning(AU)

ABSTRACT

Abstract Background The first case report on the Cannabinoid hyperemesis syndrome (CHS) was registered in 2004. Years later, other research groups complemented the description of CHS, adding that it was associated with such behaviors as chronic cannabis abuse, acute episodes of nausea, intractable vomiting, abdominal pain and compulsive hot baths, which ceased when cannabis use was stopped. Objective To provide a brief review of CHS and report the first documented case of CHS in Mexico. Method Through a systematic search in PUBMED from 2004 to 2016, a brief review of CHS was integrated. For the second objective, CARE clinical case reporting guidelines were used to register and manage a patient with CHS at a high specialty general hospital. Results Until December 2016, a total of 89 cases had been reported worldwide, although none from Latin American countries. Discussion and conclusions Despite the cases reported in the scientific literature, experts have yet to achieve a comprehensive consensus on CHS etiology, diagnosis and treatment. The lack of a comprehensive, standardized CHS algorithm increases the likelihood of malpractice, in addition to contributing to the patient's biopsychosocial deterioration and raising care costs.


Resumen Antecedentes En 2004 se registró el primer reporte de caso del Síndrome de hiperémesis cannabiniode (SHC). Años más tarde, otros grupos de investigación fueron complementando su descripción, añadiendo que éste se asociaba a comportamientos como: consumo crónico y abusivo de cannabis, episodios agudos de náuseas, vómito incontrolable, dolor abdominal, baños compulsivos de agua caliente, que remitían tras el cese del consumo de cannabis. Objetivo Presentar una breve revisión del SHC y presentar el primer caso documentado en México. Método Mediante una búsqueda sistematizada en PUBMED entre 2004 y 2016, se integró una breve revisión del SHC. Para el segundo objetivo, se utilizó la guía de reporte de casos CARE para registrar y manejar a un paciente con SHC en un hospital general de alta especialidad. Resultados Hasta diciembre de 2016 se había registrado un total de 89 casos en todo el mundo, aunque ninguno en América Latina. Discusión y conclusiones A pesar de los casos reportados en la literatura científica, aún no existe un consenso de expertos más completo sobre la etiología, el diagnóstico y el tratamiento del SHC. Asimismo la ausencia de un algoritmo integral y estandarizado aumenta la probabilidad de mala praxis, además de incrementar el deterioro biopsicosocial del paciente e incrementar el costo de la atención.

En los últimos años se está viviendo un proceso de transformación muy importante en lo que refiere a los modelos y teorías en salud mental (DSM-5, CIE-11 y las propuestas del National Institute of Mental Health son claros ejemplos). Un componente central de dicho proceso es la incorporación de los aportes de las neurociencias a la salud mental, siendo el modelo psicobiológico de R.C. Cloninger un referente fundamental. Por ello, los investigadores y clínicos de nuestra región (América Latina) deben contar con instrumentos de evaluación adecuados a esta nueva realidad. El Inventario de Temperamento y Carácter revisado (TCI-R) es la escala desarrollada por Cloninger y su equipo para la evaluación de las Dimensiones de Personalidad que su teoría propone. El presente artículo presenta los resultados preliminares del proceso de adaptación de dicha escala a la población montevideana, siendo la primera adaptación realizada en la América del Sur castellano parlante.

O presente artigo apresenta os resultados preliminares do processo de adaptação da escala para a população de Montevidéu, a primeira adaptação feita em língua espanhola na América do Sul.


In recent years important there have been important developments in the models and theories on mental health (the DSM-5, the CIE-11 and the proposals of the NIMH are good examples). A central feature in this process is the incorporation of the neuroscientific approach, with the psychobiological model of R. C. Cloninger and colleagues being a salient example. For this reason, Latin American researchers and professionals should have instruments adequate for this new paradigm at their disposal. The TCI-R is a scale developed by Cloninger and colleagues for the assessment of the personality dimensions he proposed. This paper presents the preliminary results of the adaptation process of this scale in a sample of Montevidean adults,and is the first in Spanish-speaking South America.

ABSTRACT Introduction There is a growing interest in achieving higher survival rates with the lowest morbidity in localized prostate cancer (PC) treatment. Consequently, minimally invasive techniques such as low-dose rate brachytherapy (BT) and robotic-assisted prostatectomy (RALP) have been developed and improved. Comparative analysis of functional outcomes and quality of life in a prospective series of 51BT and 42Da Vinci prostatectomies DV Materials and Methods Comparative analysis of functional outcomes and quality of life in a prospective series of 93 patients with low-risk localized PC diagnosed in 2011. 51patients underwent low-dose rate BT and the other 42 patients RALP. IIEF to assess erectile function, ICIQ to evaluate continence and SF36 test to quality of life wee employed. Results ICIQ at the first revision shows significant differences which favour the BT group, 79% present with continence or mild incontinence, whereas in the DV group 45% show these positive results. Differences disappear after 6 months, with 45 patients (89%) presenting with continence or mild incontinence in the BT group vs. 30 (71%) in the DV group. 65% of patients are potent in the first revision following BT and 39% following DV. Such differences are not significant and cannot be observed after 6 months. No significant differences were found in the comparative analysis of quality of life. Conclusions ICIQ after surgery shows significant differences in favour of BT, which disappear after 6 months. Both procedures have a serious impact on erectile function, being even greater in the DV group. Differences between groups disappear after 6 months.

El entrenamiento de la inteligencia emocional en personas mayores, ha demostrado su efectividad en la mejora de la calidad de vida. El objetivo es analizar los beneficios en salud, de un programa de estimulación cognitiva e inteligencia emocional para personas mayores (PECI-PM). La muestra estuvo formada por 28 alumnos de la Universidad de Mayores de Almería, que completaron la primera fase del PECI-PM. Para la evaluación (pre-post) de la salud, se aplicó la versión española del SF-36. Los resultados mostraron puntuaciones significativamente más elevadas, tras la intervención, en las dimensiones de salud: Función Social, Dolor Corporal, Vitalidad y Función Social. La primera implementación del PECI-PM, revela efectos positivos del entrenamiento emocional sobre la percepción de la salud, en personas mayores.


Training emotional intelligence in the elderly, it has demonstrated its effectiveness in improving the quality of life. The aim is to analyze the health benefits of a program of cognitive stimulation and emotional intelligence for older people (PECI-PM). The sample consisted of 28 students of the University Senior Almeria, who completed the first phase of PECI-PM. For the (pre-post) assessment of health, the Spanish version of the SF-36 was applied. The results showed significantly higher scores after the intervention, the dimensions of health: social function, bodily pain, vitality and social function. The first implementation of the PECI-PM reveals positive effects of emotional training on perceived health in the elderly.

ABSTRACT

Sideroxylon obtusifolium, conhecida como quixaba, é utilizada para diversos fins medicinais, dentre eles em casos de inflamações e infecções bacterianas, com isso, o estudo teve por objetivo investigar a atividade anti-inflamatória tópica e antibacteriana do extrato metanólico obtido das folhas de S. obtusifolium. A prospecção fitoquímica do extrato revelou a presença de diversas classes, entre eles: Fenóis; Taninos Pirogálicos, Flavonoides, Catequinas e alcaloides. Os polifenóis e flavonóides presentes neste extrato foram quantificados em 150,3 mg equivalentes de ácido gálico/g de extrato e 98,5 mg equivalentes de quercetina/g de extrato, respectivamente. Os resultados obtidos através dos testes de microdiluição e modulação mostraram potencial atividade antibacteriana quando associados a gentamicina e amicacina para cepas multirresistentes de Staphylococcus aureus. Para cepas de Escherichia coli, houve sinergismo frente ao antibiótico amicacina havendo antagonismo para gentamicina, o mesmo efeito foi observado para Pseudomonas aeruginosas em ambos antibióticos. Nos testes para inflamação aguda através dos modelos de edema de orelha induzido por óleo de cróton e fenol, o extrato apresentou um potente efeito antiedematogênico significativo (p < 0,05). Diante dos resultados, podem-se observar atividades biológicas de S. obtusifolium, o que condiz com seu uso na terapia popular.


Sideroxylon obtusifolium, known as "quixaba", is used for various medicinal purposes, including in cases of inflammation and bacterial infections, therefore, the study aimed to investigate the topical antibacterial and anti-inflammatory activity of the methanol extract obtained from the leaves of S. obtusifolium. The phytochemical screening of the extract revealed the presence of several classes, including Phenols; Tannins, Flavonoids, catechins and alkaloids. Polyphenols and flavonoids present in this extract were quantified in 150.3 mg of gallic acid equivalents / g extract and 98.5 mg of quercetin equivalents / g extract, respectively. The results obtained by the microdilution tests have shown potential and modulation antibacterial activity when associated gentamicin, and amikacin for multidrug-resistant strains of Staphylococcus aureus. For Escherichia coli, there was synergism for amikacin, gentamicin having antagonism, the same effect was observed for Pseudomonas aeruginosa on both antibiotics. In tests for acute models of inflammation by ear edema induced by croton oil, and phenol, the extract had a potent antiedematogenic significant effect (p < 0.05). Therefore, the results can be observed biological activities of S. obtusifolium, which is consistent with its use in popular therapy.

Los resultados obtenidos por los ensayos de microdilución han mostrado actividad potencial y la modulación antibacteriana cuando se asocia gentamicina, amikacina y para las cepas resistentes a múltiples fármacos de Staphylococcus aureus. Para Escherichia coli, solo se obtuvo sinergismo con amikacina, y se observo antagonismo para gentamicina, este mismo efecto fue observado para Pseudomonas aeruginosa en ambos antibióticos. En las pruebas de modelos agudos de inflamación por edema de la oreja inducido por aceite de crotón, y el fenol, el extracto tuvo un efecto significativo antiedematogenic potente (p <0,05). Por lo tanto, los resultados muestran actividad biológica de S. obtusifolium, que es coherente con su uso en terapia popular.

A educação vem passando por mudanças importantes e, ao longo da história, foi exercida com elevada severidade até passar por um processo significativo de evolução. Desde o século XIX, com o surgimento das ideias construtivistas, uma nova dinâmica de relação entre educador e educando se estabeleceu e foi mais além ao exigir que o educando assumisse seu papel na construção do conhecimento, e o educador exercesse a função de mediador e facilitador desse conhecimento.Já foi descrita uma grande variedade de métodos de ensino, sendo a maioria deles direcionados a pequenos grupos, embora exista aplicação de métodos a grupos maiores, tais como o Aprendizado Baseado em Equipes (TBL). Esta revisão de literatura descreve um breve percurso histórico da educação, chegando à educação centrada no estudante, bem como alguns dos métodos ativos de aprendizagem mais utilizados na atualidade.


Education has been undergoing major changes and, throughout history, has been exercised with great stringency an devolved significantly. Since the emergence of constructivist ideas in the nineteenth century, a new dynamic between educator and student has been established and went further when it was demanded that the learner assume their role in the construction of knowledge, and that the educator fulfilled the duty of mediator and facilitator of this knowledge. A wide variety of teaching methods have been described, most of which are aimed at small groups, although methods for larger groups are applied, such asTeam-Based Learning (TBL). This literature review describes a brief historical background of education, leading up to student-centered education and describes some of the active learning methods in most common usage today.

It has been established that teachers' conceptions of learning and teaching influence their instructional practices. Several authors maintain that these conceptions are based on certain implicit assumptions that give rise to different theories. Our view is that people have multiple alternative theories which they use depending on the context and the demands of the task. The main purpose of this study was to find out whether such representational plurality exists in teachers' conceptions and, if it does, whether this plurality can lead to the identification of different representational profiles. We were also interested in studying some of the teaching practice variables that might influence the nature of the representational profiles. Our results, obtained by means of a dilemma questionnaire answered by 1074 teachers from different educational levels and knowledge domains and with different ranges of experience, are consistent with the assumption of representational plurality, as they show that the same teacher may hold different conceptions that set up a conceptions profile. Moreover, the combination of the responses in each profile is not random. Teachers at the more advanced levels and with more teaching experience manifested more traditional conceptions. Furthermore, some knowledge domains were associated with certain conception profiles.


Se ha comprobado que las concepciones de aprendizaje y enseñanza de los profesores influyen en su práctica docente. Varios autores sostienen que estas concepciones se basan en ciertos supuestos implícitos que dan lugar a distintas teorías. Nuestra visión es que la gente dispone de multitud de teorías alternativas que pueden utilizar en función del contexto y las exigencias de la tarea. El objetivo principal del presente estudio es averiguar si esta pluralidad representativa existe en las concepciones de los profesores y, de ser así, si puede conducir a la identificación de distintos perfiles representacionales. Nos interesamos, asimismo, por el estudio de algunas de las variables de la práctica docente que pudieran influir en la naturaleza de dichos perfiles. Nuestros resultados - obtenidos mediante un cuestionario de dilemas realizado por 1074 profesores de distintos niveles educativos, ámbitos de conocimiento y experiencia- coinciden con la suposición de la pluralidad representativa, pues revelan que el mismo profesor podría forjar concepciones distintas que conforman un perfil de concepciones. Además, la combinación de respuestas en cada perfil no es aleatoria. Los profesores de niveles más altos y los más experimentados manifestaron unas concepciones más tradicionales. Igualmente, algunos campos de conocimiento se asociaron a perfiles de concepciones concretos.

INTRODUCCIÓN : la producción de medicamentos requiere de una exigente política de calidad que tenga la finalidad de salvaguardar los intereses del paciente, la sociedad y el Estado. Uno de los procesos más variable dentro del ciclo de vida de un producto y con requisitos muy específicos para el caso de los medicamentos es el servicio posventa. OBJETIVO: perfeccionar el proceso de servicio posventa de los productos para uso humano que comercializa el Centro Nacional de Sanidad Agropecuaria. MÉTODOS: el estudio se sustenta en la aplicación de herramientas de calidad, como son: diagrama causa-efecto, tormenta de ideas, lista de chequeo, representación de los problemas detectados y aplicación de la metodología planificar-hacer-verificar-actuar. RESULTADOS: se logró identificar a los subprocesos de atención al cliente, competencia, distribución, garantía, adiestramiento para el uso, quejas y reclamaciones, retiro de productos y farmacovigilancia como los factores principales que caracterizan al proceso. Se definieron y evaluaron los requisitos de cada uno de ellos; se obtuvo solo el 54,5 por ciento de su cumplimiento. El diseño e implementación de un plan de mejora, permitió el perfeccionamiento del proceso, con un cumplimiento del 100 por ciento de los requisitos diseñados. Destacan el rediseño de las funciones, responsabilidades, interrelaciones, entradas, salidas, recursos, sistema de procedimientos y registros, con énfasis en subprocesos tan específicos y complejos como la distribución, retiro de productos y la farmacovigilancia. CONCLUSIONES: se aportan indicadores para evaluar el desempeño y facilitar la toma de decisiones y administración más eficiente del proceso, que elevan el nivel de satisfacción de los clientes. Estos resultados contribuyen a mejorar el servicio posventa de los productos con interés comercial y al cumplimiento de los requisitos de las buenas prácticas de fabricación de medicamentos(AU)


INTRODUCTION: drug manufacture requires strict quality policies that are aimed at safeguarding the interests of the patient, the society and the state. One of the most variable processes in a product's lifecycle, with very specific requirements in the case of drugs, is the aftermarket service. OBJECTIVE : to upgrade the aftermarket service process for the human use products that the National Center of Agricultural Health markets. Methods: the study was based on the use of quality tools such as cause-effect diagram, brainstorm, checking list, depiction of detected problems and application of the methodology called planning-doing-checking-acting. RESULTS: it was possible to identify the subprocesses of attention to the client, competition, distribution, warranty, training for use, complaints and claims, product withdrawal and pharmacovigilance are the main factors characterizing the process. Requisites of each of them were defined and evaluated, but just 54.5 percent were fulfilled. The design and implementation of an upgrading program allowed improving the process and fulfilling all the planned requisites. There should be mentioned the re-design of functions, responsibilities, interrelations, inputs, outputs, resources, system of procedures and registrations, with emphasis on complex specific subprocesses like distribution, product withdrawal and pharmacovigilance. CONCLUSIONS: some indicators are provided to evaluate the performance and to facilitate decision-making and more efficient management of the process in order to raise the level of satisfaction of the client. These results contribute to improve the aftermarket service of products and the compliance with the good practice requirements in the drug manufacture(AU)

ABSTRACT

Los LF representan aproximadamente el 25% de los Linfomas no Hodgkin. Debido a su curso clínico indolente y recaídas habituales, son considerados una entidad incurable con los regímenes quimioterápicos convencionales. Aunque el advenimiento del Rituximab modificó la sobrevida global, el tratamiento de mantenimiento con este fármaco retrasa la recaída y consolida la respuesta alcanzada luego de la fase de inducción. Con una dosis semanal de Rituximab de 375 mg/m2 se demostró una acumulación progresiva de su concentración plasmática, la cual continúa siendo detectable luego de 3–6 meses de la última infusión. Existen varios esquemas de mantenimiento que varían con intervalos entre 2, 3 y 6 meses, todos ellos han demostrado efectividad. El más utilizado resulta el trimensual ya que logra mantener niveles séricos de Rituximab mayores a 25 μg/l (valor target en mantenimiento) con una duración de 2 años. Entre los efectos adversos predominan la reagudización de infecciones virales y la disminución de inmunoglobulinas (Ig) (principalmente Ig M), sin un incremento en la mortalidad. Debido a que existe una correlación lineal entre el descenso de Ig y la actividad del Rituximab, la terapéutica debería personalizarse según los niveles de Ig previos a cada ciclo de mantenimiento para optimizar la distancia entre los mismos, ahorrando costos y disminuyendo el riesgo de infección. Además, la detección del rearreglo del BCL2 al inicio y final del mantenimiento definirá la posibilidad de recaída y la necesidad o no de prolongarlo más allá de 2 años, con un probable impacto en la sobrevida global.


Follicular lymphomas account for about 25% of non–Hodgkin lymphomas. Due to its indolent clinical course and common relapses are considered incurable entity with conventional chemotherapy regimens. Although the advent of Rituximab have modified the overall survival, maintenance treatment with this drug, delayed the relapse and consolidates the response reached after the induction’s phase. A weekly dose of 375 mg/m2 of Rituximab showed a progressive accumulation of its plasma concentration, which remains detectable 3–6 months after the last infusion.There are several schemes of maintenance that ranging from 2, 3 and 6 months, all with proved effective. The mostwidely used is the three–monthly since achieved maintain serum Rituximab levels greater than 25 μg/l (target value in maintenance) during two years.Among the adverse effects predominate exacerbation of viral infections and decreased of immunoglobulins (mainlyIgM), without an increase in mortality. Because there is a linear correlation between the decrease in immunoglobulins and the activity of Rituximab, therapy should be individualized to immunoglobulin’s levels before each maintenance cycle in order to optimize the distance between them, saving costs and reducing the risk of infection. Besides, the detection of BCL2 rearrangement at the beginning and end of maintenance would define the possibility of relapse and whether or not extended the maintenance beyond two years, with a likely impact on overall survival.

Algunos estudios han reportado que los ratones hembras son más resistentes al trypanosoma cruzi que los machos. Para probar lo anterior, se realizo un estudio comparativo con cuatro cepas de T. cruzi de Costa Rica midiendo parámetros como niveles de parasitemia, porcentajes de mortalidad e histopatología, en un total de 240 ratones (160 para parasitemia y mortalidad y 80 para histopatología). Todas las cepas correspondieron al genotipo TcI. En todos los casos los ratones machos resultaron más susceptibles a la infección (P<0,001). Se observaron parasitemias hasta cinco veces más altas en los machos que, en general, mueren antes que las hembras. La cepa Oswaldo fue la que presentó una mayor mortalidad y niveles de parasitemia más altos. El tejido cardiaco fue el más afectado tanto en los ratones machos como en las hembras, con las cepas bolita y capitán el número de nidos en el miocardio fue significativamente mayor en los machos que en las hembras. Una vez concluido el análisis comparativo se eligió la cepa humana GA, para determinar su efecto en ratones machos y hembras castrados, al lado de ratones normales (150 ratones en total). Las hembras normales se mostraron como las más resistentes, con parasitemias menores que las castradas (P<0,001), y los machos normales como los más susceptibles, con parasitemias significativamente más altas (P<0,001) y supervivencias menores (P<0.01), que los ratones castrados. No se observó diferencia significativa en cuanto al número de nidos en los tejidos entre ratones normales y castrados. Se concluye que el sexo tiene influencia en la resistencia a la infección experimental por T. cruzi.


Some authors have reported that female mice are more resistant to Trypanosoma cruzi infections than male mice. In order to test this hypothesis we did a comparative study using four isolates of T. cruzi from Costa Rica in a total of 160 mice, and observed several parameters such as: parasitemia levels, percent mortality and histopathology. All isolates were identified as genotype TcI. Male mice were more susceptible to infection (P<0.001). Parasitemias reached levels up to five times higher in males and shorter survival periods were observed in males than in females. The Oswaldo isolate showed the highest parasitemias and mortality rates. The heart tissue was the most affected in both males and females. In two isolates the number of parasites in the heart was significantly higher in males than in females. In separate experiments, the human GA isolate was selected in order to observe its effect in castrated male and female mice. Non-castrated females were the most resistant, with lower parasitemias than castrated females, and non-castrated males were the most susceptible with higher parasitemias (P<0.001) and lower survival periods than castrated males (P<0.01). There was no significant difference in the number of parasites in tissues between normal and castrated mice. The final conclusion is that there is a marked sex influence in the susceptibility of mice to T. cruzi experimental infections.

Este texto es producto de una investigación iniciada en el año 2006, que tuvo por objetivo recuperar la memoria y la experiencia de los actores involucrados en la creación y desarrollo del Instituto de Medicina del Trabajo (IMT) entre los años 1973 y 1974 en la Facultad de Medicina de la Universidad de Buenos Aires. Para ello, utilizando el recurso de la historia oral, se realizaron trece entrevistas a los principales referentes del Instituto, se efectuó un relevamiento documental en archivos personales de los miembros del IMT y en archivos públicos. El IMT, constituyó una experiencia innovadora en el campo de la salud laboral que cuestionó las categorías y prácticas en higiene, seguridad y medicina del trabajo dominantes en la época. Los principales resultados demuestran el carácter innovador de la experiencia y cómo el IMT logró formar un discurso propio, alternativo y autónomo que se convirtió en referente para la construcción de un pensamiento crítico en la relación salud y trabajo que trascendió las fronteras del país y que hoy forma parte del discurso de numerosos centros de docencia e investigación de diferentes países. Las fuentes recuperadas en el curso de esta investigación forman parte del Centro de Documentación Pensar en Salud del Instituto de Salud Colectiva.


This text is the product of an investigation initiated in 2006 with the objective of recuperating the memories and experiences of those involved in the creation of the Instituto de Medicina del Trabajo (IMT) (Institute of Occupational Medicine) between 1973 and 1974 in the Faculty of Medicine of the Universidad de Buenos Aires. Thirteen interviews with referential figures of the Institute were conducted, using their oral histories as a resource, and a survey of documents from personal archives of members of the IMT and from public records was carried out. The IMT constituted an innovate experience in the field of occupational health, questioning the categories and practices in hygiene, safety, and occupational medicine prevalent at that time. The primary results demonstrate the novel character of the experience and the way in which the IMT was able to create its own autonomous and alternative discourse that prompted the development of critical thought in relation to health and work. This capacity transcended the limits of the country and still today influences the discourse of research and educational centers from many different countries. The sources collected as part of this investigation now form part of the Centro de Documentación Pensar en Salud (Thinking about Health Documentation Center) of the Instituto de Salud Colectiva.
